1. Apply Off-line
- Download application you need for Driver's Document.
- Print out your application.
- Sign your application.
- Attach 1 (one) passport size color photo to the application.
- Make Certified Check or Money Order payable to "IADC" Inc.
- Make a copy of your national driver license.
- Send your application with photo, signature and copy of your national driver's license
to our office.
